Background: In Iran 46% of the overall mortality rate is related to circulatory diseases and cardiovascular diseases (CVDs) are increasing year by year. Such diseases force severe economical loss on the body of the Iranian society as well. Completed studies have proved that CVD risk factors also have a high prevalence in the society in a way that hypertension is 18%, hypercholesterolemia is 33%, hypertriglyceridemia is 44%, obesity is 58% and diabetes is 7%. A non-healthy diet and lack of physical activity are also prevalent in Iran. Considering the mentioned situation, it seems necessary to operate a comprehensive integrated community-based program in the society using population and high risk strategies with the aim of reducing the common risk factors of noncommunicable diseases. The National Program of Healthy Life Style for Non-communicable Diseases Prevention and Control is therefore being organized and carried out in three cities (as demonstration and reference areas) for reaching suitable and feasible interventions in this respect. |

Aims & Objectives: Main Objectives: To reduce CVD mortality and morbidity. To reduce the incidence and prevalence of NCD and its risk factors. Specific Objectives: To promote healthy lifestyle particularly in the areas of tobacco use, diet and physical inactivity and reducing CVD risk factors in the community. To provide the experience necessary to extend the project into a nation-wide program. Overall Objectives: To test different methods of cardiovascular disease prevention applied simultaneously and systematically in the same community. To evaluate the feasibility, effects and other experiences of a major, comprehensive effort. To be a source of public and professional inspirations, visibility, training and other intellectual resources. |
Launch site & Samples: The population evaluated in this study involve all citizens of rural & urban areas of Isfahan, Najaf-Abad and Arak as pilot cities. The population are above 19 years of age, the selection is according to the distribution of sex and age through the society. The whole sample is 12600 ( meaning 6300 from the intervention area and 6300 from the non-intervention-area ). Sampling is done by clustering method. This study also evaluate, the risk factors among ( 2000 ) children 4-18 years old, the knowledge, attitude and practice (KAP) of 2000 health personnel and 2000 CVD patients regarding risk factors and diseases as well. Stages of the study: This study has three stages: 1- The first stage: Evaluation of the present situation: At this stage, both intervention and nonintervention populations are evaluated. Main studies include determination of risk factor prevalence using the MONICA questionnaire and taking blood tests for adults and children, determination of CVD prevalence using the ROSE questionnaire, electrocardiography analysis using Minnesota coding, determination of CVD & stroke mortality rate using mortality & morbidity statistics and finally determining the level of knowledge, attitude and practice of the population, health personnel and CVD patients. 2 - The second stage: Intervention stage: At this stage, varied comprehensive interventions are done in the intervention cities which mainly consider risk factor control & lifestyle modification with the involvement of most Governmental and Non-Governmental Organizations. It is constituted of 8 big interventional projects. 3 - The third stage: Evaluation of the situation after intervention: At this stage, studies of the first stage is repeated on the two intervention & non-intervention groups, then compared to the first stage results in order to test the effect of the interventions used.
